I don't think these answers really are accurate because he was a surgeon. We didn't need to talk. We didn't need to. This was not an office visit. So for what I had done at Epiduro, I think he was great, but these answers to these questions really don't apply. Thank you.

I want a refund of my co-pay of $95. I waited MONTHS for this appt for myself and my sister and it was a joke. Dr said he can only do surgery or narcotics. What about pain patches, rubs, anything else?! I asked him about the website and he said that he DIDNT MAKE THE WEBSITE. REALLY!? never have I ever been treated SO POORLY and making me feel like I was inconveniencing his day. PIH - DO BETTER with your Dr. THis is horrible.

Dr said that all he can offer is surgery or narcotics. When I referenced the PIH Pain Management Website, he said I didn't make that website I only do surgery or narcotics. When I asked about Botox injections, he said yes if you spastic - I said I am. Didn't offer any other treatment and was irritated that Dr. (name removed) referred me. Dr said that he's not Dr. Pangs assistant to write RX for ibuprofen. Waste of time and its very difficult for me to get there being disabled. VERY BAD EXPERIENCE. I WILL NEVER GO BACK.

Procedure of epidural at the same-day surgery center went very well I couldn't have been treated better

I arrived early for my appointment. No effort was made to get me in earlier. No effort was made to update me on how soon I would possibly get in to see the doctor. I had to physically get up from my seat while I was in pain (3x) to check on my appointment.

Dr Hafezi is professional and treats me with respect. I trust him with my health and care.

The ladies out front in the front office, before you get back to the doctor, were great. I love them. They helped me out a lot. The lady who took me back into the doctor, not so much. A little bit rude. Don't know her name, though. And the doctor kind of blew me off a little bit when I was telling him what was going on with me. But for the most part, he listened. He could have paid more attention. That's it. Thank you.

Five weeks, I was in pain from my waist up to my neck. Took an MRI, MRI, doctor looks at it and says I need an epidural. My hands, my wrist, my forearm, my elbows, all the way up to my shoulders, to my neck. I could not move it. I had the epidural done on Friday the 19th, and I woke up this morning. I couldn't even move my arms, the same exact severe pain. So whatever Dr. Hafezi has given me epidurals before on my lower back, and they worked. Now, this one has not worked, and I am in excruciating pain. I try to get a hold of him today to find out what I can do or what happened, and I can't. My name is (PHI removed). I always feel as though I'm not rushed. Dr. Hafezi always makes me feel comfortable and not rushed

The doctor was very respectful, kind. He explained the procedure and did a fabulous job. It was a procedure I had already had done prior and it was less painful. He made me feel very comfortable and I would definitely refer him to any other family members or friends. And I really appreciate the courtesy that he gave me with any follow-up that I may need to have.

Only thing is, he is booked and hard to get sooner appointments. But I kept calling until I got lucky and got a sooner appointment to see him, someone had just cancelled their appointment when I called! When pain is bad enough I try to get seen sooner. Sometimes this doesn't work.

I would like to be able to see him less frequently than every three months, because the wait between appointments is too long. I would prefer to schedule appointments at least every two months, but three months is still too long. That's my only complaint. Otherwise, I'm very grateful. He's been a good doctor.

The young woman at the counter across the hall, named (name removed) (at least that's the name she gave me), was very rude and bossy. She also discriminated against me because I don't speak English, and even though I reported the incident, no one did anything about it. I felt offended, discriminated against, and humiliated. I'm going to do everything I can to change my doctor and office. I'm also going to file a complaint with the county health department.

He was in a big rush and he was in a hurry and he wasn't listening to me as a patient. He filled my prescription, which I'm grateful for. But no, I would not, I don't want him to wait on me again because he won't, he doesn't listen. He's in too big of a hurry. Thank you. Yeah. Yes, I got it. Get your pad from him, (PHI removed).

I was treated with respect from the second I walked into that office to say that I was there for my appointment. Doctor was really kind and knew that I had concerns and he took his time. He did the test. He explained everything with the test. I couldn't have been happier. I didn't know what to expect and I was very, very happy with everything, all the good service that I received. Everybody was nice and kind and knowledgeable in the office. The only problem I have is that when I was in the middle of a crisis and I contacted them, I didn't get a call back and it took me four days to receive the medication that I needed four days ago. It was very frustrating and I went through a lot of pain because of that. Thank you.

That it's worthless that we have to register online, check in online, go through that whole screens and then see commercials that we don't even want to see. And then you have to come in and stand in a long line. Dr. Hafezi's office and the other offices, they're too long. I mean, I lost my appointment. It was at 3:15 and I, or 3:30, and I got there at 3:35, and they didn't want to take me. I had to argue with the girls. So, I mean, that line is always long. And if we've already registered, why do we have to stand in again and check in? Other than that, they just need to be a little more courteous, the front desk. Thank you. Bye.

I am 56 years old. During the last 18 years I have required medical attention regularly. Never have I had such great care from Receptionist all the way to Dr's. Simply a great environment. Completely pleased. Thank You.

This is a very inflexible pain management doctor who only does epidurals. I would not suggest anything invasive for people that come in with chronic lifelong pain, who is only concerned about his own paycheck and looks unprofessional, did not seem like a doctor, was wearing sandals and socks, and I could not take him seriously. He walked out of the room, never completed our visit, and never came to a conclusive resolution for a treatment plan. Thank you.

Dr. Hafezi was very patient in listening to all my pain problems. He explained which medications and treatments would be best for my condition.

I would like for the medical assistant of the doctor to be more kind and to acknowledge the patient. She doesn't look at you and she talks really fast like she's a robot. I didn't like that. It didn't seem professional. She doesn't look at you, no acknowledgement. She needs to improve her customer service.

Everything was great with the doctor. I received excellent care there; in fact, I arrived late because the hospital reception staff weren't helpful and I was going to miss my appointment, which is very important to me. But in every other respect, the doctor was very attentive, and I would recommend him 100% to my family and friends. However, I have another comment: the appointments are very long. When you want to see the doctor, they give you appointments months in advance. So I think they should improve their system by scheduling appointments further in advance, because they are very long. Thank you, that's my comment.

This provider doesn't seem to care about the pain levels that I try to discuss with him and he seems to always think that or tell me that these pains are in my head or that they're nonexistent and that I can just treat everything with a Tylenol. I try to explain to him otherwise and he just won't listen to me and I'm very, I'm just, it's just not right. He's a pain medication doctor. He's supposed to talk with me about how to deal with my pain, not just be a gatekeeper to narcotics that I may be taking. It's a very serious problem. I'm in extreme pain and I'm supposed to take a Tylenol.

Overall, everything was excellent, truly excellent. Thank you.

I think the CNA that took you back to the room could have been more nicer. She was very cold-ish and off. She was not a make you feel welcome or anything. That's what I didn't like. But everybody else was on top of things. They were great, except her. Thank you.

I'd like a follow up

Dr. Hafezi has always been attentive and thorough at every appointment. His recommendations for care procedures are always clearly explained and he allows for concerns or questions for further clarification I might have. I have been fortunate to have him care for and manage my lower back concerns throughout the years.

Yes, I was very early for my appointment and they were able to get me in in a real timely fashion. Although my follow-up appointment is three months out, I don't know if that's common, but I thought it was a little long.

The Dr. Looked sty chart so he was familiar with m problem since he had not seen me since 2012. We discussed past and present and decided on a health plan. I was very pleased with my visit.

I was very happy with the doctor and the speed to move onto the next steps to help with my pain. Only problem trying to get a next appointment to follow up on my results after the tests nearly impossible have to see a PA instead hopefully that will go well.

My appointment was at 3:30 I transcribed in my calendar in error. I arrived at 3:50 and Dr Harazi still honored my appointment for my Nerve Conduction testing. I am very grateful to him. Thank you again Doctor Harazi!

the parking at the facility is horrible. it took me 30 minutes to find parking and i had to walk from the far end of the lot. (i am having a problem with pain when i walk). this is the second time that it has been this bad. something needs to be done. i was almost late for my appointment but i make a point to trying to be early.

I love the fact that you can pre check all your information and pay in advance, but I hate the fact that sometimes you have to wait in line over five minutes to let the front desk know that you are there. Another issue is the parking situation. Sometimes you can get out of the parking lot pretty fast. But other times, my last visit, you have to wait in line almost 10 minutes because the line doesn't move and there's a long line of cars trying to get out .

He was clueless about MrI and treatmeant I had done from the beginning.

The doctor answered all my concerns gave me advice what to do if I got worse he had x-rays taken to make sure that there was no other problem & told me to see the surgeon that was in charge of my case

everything was good except the time you have to wait to get an appointment when you are in pain and then you have to wait for a call to get an appointment to go to another place to finally get a shot for pain. it doesn't seem to matter if you are in pain, just wait some more and suffer like you have been. not fun

DR Hafezi and his staff were courteous, professional and helpful. The doctor listened carefully, asked questions and then explained what the options were to address the issue. He went further and discussed what follow up actions might be available if the initial procedure was not successful.

This visit stemmed from a referral from Orthopedic Department. It appears the referring physician was not aware of this doctor's limitations. Pain alleviation in terms of medication was limited. Fifth teen pills for 30 days, meaning I have to control my pain to not exceed the ability for the dose restriction to alleviate pain.

I was the only person in line. There were 4 staff members talking to each other (not work related) and it took them a few minutes to acknowledge me or help me.

Dr. HAFEZI MAKES ME FEEL VERY COMFORTABLE AT ALL MY VISITS.

Dr Hafezi was attentive and listened carefully. He made good suggestions to address my concern and plan for future health care. I was very happy with my visit and the results.

Waiting line too long makes it difficult for disabled folk

My appointment was for 7:45 in the morning and I did not see the provider until 8:30 and I believe I was either his first or second patient on the list as he arrived going on to 8 o'clock. Thank you.

Hello. As I tried to call somebody to complain about my visit, somebody was supposed to call me back on the medical office side. I got a hold of the hospital side. I was extremely unhappy with Dr.(name removed) after many years, but my last appointment on Thursday, or I... I don't remember, was extremely unsatisfying. He was unconcerned. I had to follow him down the hallway to ask questions. It was not even worth my trip there. And very disappointed. I asked for somebody to call me back because I was registering a complaint.

Well, he didn't even check me. He just listened to what me and my son had to say, but he didn't even check my spine or nothing. And then he just gave the option of giving me an injection on my spinal cord, but not there in the hospital. And after that, the man just said, okay, that's it. And he just went about his way and we went about hours. That's about it. So I really didn't get that because I thought he was supposed to check me and help me for my pain, but it didn't happen. Okay, thank you.

Dr Hafezi took time to go over my medical history and discuss any questions and concerns that were bothering me.

This is nothing against Dr. Hafezi. I like him as a doctor. I trust what he has to say. My problem is all doctors, all healthcare people do not give you enough time. 10, 15 minutes in the office is not enough time to go over everything. We need to have more time in the office. We spend more time waiting in the waiting room for the doctor than we do inside when we're talking to the doctor. And that goes with every doctor there. That's all I have to say. Thank you.

The only challenge or difficulty that I have is rescheduling. I was asked to reschedule and it's three months from now. I would hate to have cancer and be told that I couldn't see my doctor for a follow-up for three months. That's very frustrating. It continues to be more frustrating than trying to schedule appointments, trying to do this, trying to do that. There's no opening and it's not for months out. So that's fixed. It's going to be very hard to give you any proper good rating.

Dr. Hafezi is an awesome medical doctor. He is compassionate and understanding to my medical needs. I highly recommend him for pain management.

I'm usually very happy with this location. I just ask that they respect the patient's time. My allotment was for 7:45am. The office looked closed when I arrived at 7:35ish. The young lady at the front desk looked as if she arrived and I had to wait until 7:45 to check in because she was busy opening her station. When I finally got in the room at 8:30ish, the provider hadn't set up any of the tools he needed until he entered the room. I feel they should have arrived at least 30 minutes before the first appointment in order to be ready to receive patients. Everyone was kind but it was an inconvenience to have to wait for them to do all the prep work that should have been done before the first patient appointment. I guess it's like going to a restaurant and waiting for the staff to clean up from the night before while you sit at the table. Then have to wait for them to prep the kitchen before they could take your order.

Very disappointed with the visit. Was offered no help and no options.

The Dr. Cut me off, whenever I tried to speak with him, about the problems I was having, and what was causing the pain, in my knees and feet. He seemed in hurry to get the Appointment over with and just make sure that I received more medication!

The office was so busy, but it was like a well oiled machine with getting everyone taken care of. I was impressed watching the check in process.
