DR Hafezi and his staff were courteous, professional and helpful. The doctor listened carefully, asked questions and then explained what the options were to address the issue. He went further and discussed what follow up actions might be available if the initial procedure was not successful.

This visit stemmed from a referral from Orthopedic Department. It appears the referring physician was not aware of this doctor's limitations. Pain alleviation in terms of medication was limited. Fifth teen pills for 30 days, meaning I have to control my pain to not exceed the ability for the dose restriction to alleviate pain.

I was the only person in line. There were 4 staff members talking to each other (not work related) and it took them a few minutes to acknowledge me or help me.

Dr Hafezi was attentive and listened carefully. He made good suggestions to address my concern and plan for future health care. I was very happy with my visit and the results.

My appointment was for 7:45 in the morning and I did not see the provider until 8:30 and I believe I was either his first or second patient on the list as he arrived going on to 8 o'clock. Thank you.

Hello. As I tried to call somebody to complain about my visit, somebody was supposed to call me back on the medical office side. I got a hold of the hospital side. I was extremely unhappy with Dr.(name removed) after many years, but my last appointment on Thursday, or I... I don't remember, was extremely unsatisfying. He was unconcerned. I had to follow him down the hallway to ask questions. It was not even worth my trip there. And very disappointed. I asked for somebody to call me back because I was registering a complaint.

Well, he didn't even check me. He just listened to what me and my son had to say, but he didn't even check my spine or nothing. And then he just gave the option of giving me an injection on my spinal cord, but not there in the hospital. And after that, the man just said, okay, that's it. And he just went about his way and we went about hours. That's about it. So I really didn't get that because I thought he was supposed to check me and help me for my pain, but it didn't happen. Okay, thank you.

Dr Hafezi took time to go over my medical history and discuss any questions and concerns that were bothering me.

This is nothing against Dr. Hafezi. I like him as a doctor. I trust what he has to say. My problem is all doctors, all healthcare people do not give you enough time. 10, 15 minutes in the office is not enough time to go over everything. We need to have more time in the office. We spend more time waiting in the waiting room for the doctor than we do inside when we're talking to the doctor. And that goes with every doctor there. That's all I have to say. Thank you.

Everyone there was polite and professional.

The only challenge or difficulty that I have is rescheduling. I was asked to reschedule and it's three months from now. I would hate to have cancer and be told that I couldn't see my doctor for a follow-up for three months. That's very frustrating. It continues to be more frustrating than trying to schedule appointments, trying to do this, trying to do that. There's no opening and it's not for months out. So that's fixed. It's going to be very hard to give you any proper good rating.

Dr. Hafezi is an awesome medical doctor. He is compassionate and understanding to my medical needs. I highly recommend him for pain management.

I'm usually very happy with this location. I just ask that they respect the patient's time. My allotment was for 7:45am. The office looked closed when I arrived at 7:35ish. The young lady at the front desk looked as if she arrived and I had to wait until 7:45 to check in because she was busy opening her station. When I finally got in the room at 8:30ish, the provider hadn't set up any of the tools he needed until he entered the room. I feel they should have arrived at least 30 minutes before the first appointment in order to be ready to receive patients. Everyone was kind but it was an inconvenience to have to wait for them to do all the prep work that should have been done before the first patient appointment. I guess it's like going to a restaurant and waiting for the staff to clean up from the night before while you sit at the table. Then have to wait for them to prep the kitchen before they could take your order.

Very disappointed with the visit. Was offered no help and no options.

The Dr. Cut me off, whenever I tried to speak with him, about the problems I was having, and what was causing the pain, in my knees and feet. He seemed in hurry to get the Appointment over with and just make sure that I received more medication!

The office was so busy, but it was like a well oiled machine with getting everyone taken care of. I was impressed watching the check in process.

It wasn't what I expected. But at least I got x-rays out of it. So hopefully the x-rays will show something that I can be referred to somebody else. Thank you.

From the moment you make the appointment you see Dr. (name removed), you explain the problem to him, they quickly schedule the appointment to go to the procedure. The day of the appointment arrives, Dr. Hafezi and the Anesthesiologist pass by the room where you are waiting to be taken to the Surgery room where all the staff are very respectful and friendly. When you wake up you are already in the recovery room, they ask you how you feel, they give you water or juice, they help you get dressed, they put you in a wheelchair and from there they take you to your car, everyone is friendly, thank you for everything.

Great guy and staff. Keep up the good work. Although the lobby was full of patients to be seen they handled it greatly.

I wish I am hoping for a fast end to my problem I can't wait too long I'm in excruciating pain

Dr. Hafezi was great. He explained my condition, but that I really need to see an Orthopedic doctor. He still took x-rays and showed me where the problems are, but said he really needed an opinion from orthopedic as to how to proceed with pain management. He in an ASAP referral to orthopedic, and I have an appointment. But Dr. Hafezi took the time to explain everything to me, even though I might need another specialist to deal with my issues.

I've had pain for six years. I've had other doctors tell me it's piriformis syndrome. But because the pain doesn't fully radiate down to my foot he won't schedule me for a cortisone injection. I get he wants to be conservative but I'd rather try it and potentially not feel pain than spend more time and money on other potential treatments. I've tried everything else pt, acupuncture, chiropractor, pain meds, muscle relaxers. Nothing works. So frustrated. I had to see another doctor to set an order for pain management and when I get here I still can't get what I'm asking for.

I was feeling like I was in my deathbed and my Dr brought me back to life . I so forever grateful to him that I have painless days and nights thank you so so much

Dr Hafezi is awesome I went from hardly able to work to now functioning like I use to, with injections and medication physical therapy, I got my life back!! And his staff is just as amazing!!

For the most part, this was a waste of time.

I felt like the Dr. had already decided what was wrong before he ever saw me. It wasn't until I told him, I was sent to him because of a pinched nerve, then he looked at the MRI. And agreed with me about the pinched nerve.

My wait was long due to a patient that was with the doctor but the front desk informed me of a problem & that the doctor would be with me asap

Dr. Hafezi was very nice and professional, but he didn't explain the epidural procedure at all to me, which has not been scheduled yet. Just wished he had. But I should have spoken up and asked him to explain it.

Testing equipment was not working in the room. (blood pressure, etc.).

I went to this provider office and I was not seen for approximately 35 to 40 minutes. He was not sensitive at all with my medical condition. I was referred to him for pain management because I had spinal cord surgery. And his response to me was that why was I referred to him because there's nothing he can do for me. So I'm going to be calling my spinal cord specialist tomorrow, Monday, the 31st and tell them that they can refer me to another pain management doctor. This doctor was very insensitive. He pretty much told me that he can't do anything for me. My telephone number, (PHI removed). My name is (PHI removed). Thank you.

We have a problem with appointments. We call appointments, very hard to reach them. We don't get an appointment on time. That's only the main things I have to say. All providers is good, no problem. But the main thing is you have to approve appointments. Thank you.

Dr. Hafezi is a good doctor and some of the questions that was asked me I just hurried up and answered because I thought maybe two might have been better than one but it actually was four that I was supposed to push and Dr. Hafezi I wouldn't trade him for anyone in PIH family. So forgive me if I didn't push all four. Dr. Hafezi I love you and you have a blessed, blessed life.

Always a good visit with this doctor/office/staff. The whole group could give lessons on niceness.

I only saw the doctor just before he did my procedure, so I didn't have too much time with him. I saw Dr. (name removed) in the office instead, but I liked him very much anyway.

So my experience overall was not that great in regard to my checking in process. I assumed that checking in on my mobile device was enough because I didn't get any other instruction on my cell phone that I had to check in also with the front desk. I ended up waiting about an hour before I was seen. So maybe if the instructions on the cell phone can be more specific as to you're checking in on the phone, but that's not the entire checking in process. That's just from what they told me. It's just pre-registering. It's not actually checking in. So yeah. Thank you.

So, currently, I have difficulty submitting the FMLA medical certification to my insurance company. They're requesting it, and I was hoping to, you know, get this submitted or filled out by this provider. Unfortunately, that wasn't the case. The provider was very hesitant to even, you know, try to find a solution for me. I don't know. I just wasn't really expecting that very much. Everything else was well, except for that reason, and I'm just questioning the form itself now, you know, and just hope he could have guided me... could have guided me furthermore to what to do or what to do or who to ask.

I like the way he explained things to us and how he had to go about things that pertain to my illness. So, yes, I would go back to him. I'm still going back to him anyway. Okay?

The whole appointment I had with the provider was about maybe 10 minutes long. He did not really ask me how I was feeling or exactly what led me to make the appointment. Within about six minutes into the conversation, he wanted to prescribe me antidepressant pills.

The Dr is great.

I was seen at the outpatient center for a procedure. Everyone was so friendly and professional. From the receptions to the nurses and the doctors.

Delay in scheduling procedure was due to excessive delay in obtaining prior authorization from medical group. (Insurance said they were not involved in decision process.)

I appreciate and was very comfortable with Dr. Hafezi. He is an excellent doctor. He thoroughly checked me, my nerves, he's explained things to me and the technology that he has is outstanding. He's an outstanding doctor.

The doctor barely spent any time with me, didn't listen to what I needed to explain, treated me as if he didn't believe me, didn't help me at all, the appointment was basically a waste of time, I went into the appointment full of hope and left feeling let down and defeated
