Healing a Hidden Condition: Bakersfield Man’s Cerebrospinal Fluid Leak Journey with PIH Health

For years, Bakersfield resident Michael Kerns struggled with recurring ear issues. It started as what he thought was swimmer’s ear and gradually worsened, leading to infections, hearing loss, and persistent discomfort. After multiple visits to local ear, nose, and throat (ENT) specialists and his internist, Michael was referred to PIH Health House Clinic, where he met Kevin Peng MD, a neurotologist or ear surgeon, specializing in complex inner ear conditions.
Dr. Peng initially treated Michael with prescription medication, which helped temporarily. However, about a year later, his symptoms returned—this time with fluid drainage from his ear, increasing head pressure, and further hearing loss. Concerned about a potential cerebrospinal fluid (CSF) leak, Dr. Peng ordered a CT scan, which confirmed the diagnosis.
A CSF leak occurs when the fluid surrounding the brain escapes through a hole in the skull base. If left untreated, it can lead to serious complications, including infections like meningitis. “Michael’s case was a textbook example of how a CSF leak can go undiagnosed for years,” explains Dr. Peng. “Once we identified the issue, we were able to repair the leak and restore the integrity of his skull base.”
On February 14, 2024, Dr. Peng performed a four-hour surgery at PIH Health Good Samaritan Hospital. The procedure involved reconstructing the brain’s protective lining and sealing the leak to prevent further fluid loss. Michael spent two days in the hospital before continuing his recovery at home.
“The care I received was outstanding,” said Michael. “The hospital team was phenomenal, and Dr. Peng gave me complete confidence in the process. My hearing has improved, and I can finally swim and travel without discomfort.”
Now fully recovered, Michael has resumed his consulting work in the oil and gas industry. He encourages others to seek expert care: “If you have a serious inner ear issue, go to PIH Health House Clinic. They know what they’re doing!”
