Critical Limb-Threatening Ischemia (CLTI) is the most severe form of peripheral artery disease (PAD). It happens when arteries in the legs and feet become so blocked that blood cannot deliver enough oxygen to keep tissues healthy. Without proper blood flow, wounds won’t heal, pain can become constant—even at rest—and infection or gangrene can develop.

Common Signs and Symptoms of CLTI

You may be at risk for CLTI if you have:

  • Nonhealing wounds or ulcers on the foot or toes
  • Severe leg or foot pain that may worsen at night or when resting
  • Skin changes such as discoloration, thinning, or shiny appearance
  • Cool or cold feet compared to the rest of the body
  • Numbness or tingling in the legs or feet
  • History of severe arterial blockages or advanced peripheral artery disease

People with diabetes, kidney disease, or a history of smoking are at higher risk for developing CLTI.
