Who We Serve
Home Health is short-term, physician-directed care designed to help a patient prevent or recover from an illness, injury, or hospital stay. PIH Health Home Health serves large portions of Los Angeles and Orange Counties. Talk to your doctor about home health if you or a loved one want to continue living independently while receiving care for an illness or injury.
Prompt & Superior Care
Home Health services are prescribed by a physician to help with rehabilitation after an illness, injury, hospital stay or surgery, or to help manage a chronic medical condition. Patients are seen within 48 hours of referral and care is available seven days a week until 10 pm. Home visits are provided by PIH Health Registered Nurses, Therapists, Social Workers and Aides until your customized plan of care is complete.

In-Home Palliative Care
For patients living with a serious illness or chronic condition, home health may be supplemented with palliative care. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family. Sometimes, it’s used in conjunction with treatments intended to cure. The kind of care you receive is dependent upon your goals of care.
Palliative care providers work with your primary care doctor and your specialists to make sure your medical care matches your goals for your body, mind and spirit. You can also ask them to help you make future plans around your health and medical care. You may work with palliative care providers in your doctor’s office, your home, a hospital, a nursing home or hospice.
