Cardiac Rehabilitation

Cardiac rehabilitation can help you get back on your feet, learn to manage your heart condition and support healthy living. Your doctor may recommend cardiac rehab after a heart attack, a heart-related procedure or heart surgery. Cardiac rehab also can help you better live with a chronic condition like congestive heart failure (CHF), stable angina or coronary artery disease.

Our care team includes healthcare professionals from many specialties. In addition to your primary care doctor and your heart doctor (cardiologist) who refer you to us, your rehab team may include our:

  • Cardiac rehab nurses  
  • Exercise specialists (exercise physiologists)
  • Registered dietitians
  • Pharmacists
  • EKG and telemetry monitor technicians
  • Medical social workers

Comprehensive Cardiac Rehabilitation Program

We want to help you slow down or stop heart disease in its tracks. Our comprehensive three-phase rehab program provides everything you need to return to the best possible level of function, including:

  • Supervised cardiovascular exercise three days each week
  • Nutrition counseling with a Registered Dietitian (RD) to help you learn to prepare healthy foods
  • Social services to support you emotionally and help you better handle the stresses of daily living
  • Education and information to help you manage high blood pressure, high blood cholesterol or diabetes and learn to:
    • Stop smoking
    • Set up and stick with an exercise program to support a healthy life and help you maintain your gains
    • Identify and manage your risk factors for heart disease
    • Safely manage your heart medications

Three-Phase Rehab Program

Our Phase I rehab activities start right after surgery while you’re still in the hospital. Our goal is to get you back to your life as quickly and safely as possible.

You’ll participate in Phase II rehab as an outpatient. Our 12-week program meets for an hour of cardiac monitored supervised exercise three times each week.

After you complete the Phase II program, Phase III cardiac rehab is available on a self-pay basis for as long as you want to use it. It consists of hour-long exercise sessions up to three times a week if you choose, in the same location and with the same familiar staff who worked with you during your phase II program, all of which will help you maintain your gains.

Advanced Heart Monitoring

We use the latest heart monitoring (telemetry) system to help us keep an eye on your heart health while you are in Phase II. Continuous monitoring helps us track your progress and make adjustments to your plan. We also report all the information to your doctor and healthcare team. During Phase II rehab, you’re monitored throughout the program. When you stay with us into Phase III, we monitor you every three months or as needed.
