Transitional care helps you or your loved one make the change from hospital to home.

Help to Get Home in Good Shape

You’ve recovered enough that you don’t need full hospital care. But you’re not yet able to go home. That’s when you may benefit from a short period of in-between care, also called transitional care.

Special Medical Services

At PIH Health, you can take advantage of special medical services offered in our Transitional Care Unit (TCU), named for the R.C. Baker Foundation. We create a personal plan for you that may include:

  • Skilled nursing
  • Rehabilitation – Physical therapy, occupational therapy and/or speech therapy
  • Nutrition counseling
  • Wound care management
  • Pain management
  • Intravenous (IV) medications
  • Prosthetics, orthotics, assistive equipment
  • 24-hour access to all hospital support services
  • Spiritual care
  • Visits, when possible, from our volunteer pet therapists, musicians and artists
  • Family and caregiver education and training to support your safe return home
  • Information and resources to help you plan for a successful life at home

Focus on Rehabilitation

We focus on your rehabilitation because we want you to be able to take care of yourself when you get home. Friends, family and caregivers are always encouraged to be part of your recovery team. We all work together to help you or your loved one regain as much ability as possible in areas like:

  • Strength and stamina
  • Flexibility and coordination
  • Balance and movement
  • Thinking, speaking and swallowing
  • Personal care and hygiene

We can also arrange for home healthcare services, help you learn how to take care of yourself when you’re at home and teach your loved ones how they can support your recovery.
