What is Post-Acute Care?

Post-acute care refers to a range of medical care services that support the individual’s continued recovery from illness or management of a chronic illness or disability.

Post-acute care includes rehabilitation or palliative services that beneficiaries receive after or in some cases instead of, a stay in an acute care hospital. Depending on the intensity of care the patient requires, treatment may include a stay in a facility, ongoing outpatient therapy, or care provided at home.

PIH Health offers the following post-acute care services:

  • Inpatient Rehabilitation (17 bed program located at PIH Health Hospital – Whittier)
  • Outpatient Rehabilitation (Locations at PIH Health Hospital – Whittier, PIH Health Hospital – Downey and in La Mirada)
  • Skilled Nursing (33 bed program located at PIH Health Hospital – Whittier)
  • Home Health (Branch offices located in La Mirada and Arcadia)
  • Palliative Care
  • Hospice
  • Outpatient Social Work

By offering the entire post-acute continuum of care PIH Health helps ease the transition between healthcare providers by better aligning our physicians, hospitals and post-acute care settings according to the patient’s needs. This collaboration is designed to streamline the transfer of care and reduce hospital readmissions.
